Physical Evaluation Form

Prior to playing any sport at Montera, the OAL physical evaluation forms must be completed. If your student does not have the medical forms in time, they will NOT be allowed to start conditioning. For the 2024-25 year, the physical must be done after June 1, 2024.

Sports Registration Update:  

All physicals need to be dated on or after July 1st. 

1. Montera needs a copy of the Pre-participation Physical Evaluation - Medical Eligibility Form for the school nurse to keep on file.
2. Families must create an account on the Arbiter website. Please follow the instructions to download and upload the correct form.
