Registration & Requirements

STEP 1: Athletes must register with Think Together After-school  Program to join Montera OAL sports teams.

All student-athletes must be registered with the After-School Program to participate in any of Montera's OAL sports teams.

Note: There is no fee for Montera student-athletes, but a completed application must be on file for the school year.

Online registration is at https://www.chooseousd.org/.

• Most caregivers should already have an account on www.chooseousd.org, because this is the website where you enroll in OUSD. 
• Once logged in, you may need to confirm the information on your Guardian Account.
• When you get to "My Dashboard" you should see your student listed, and a "Start Application" button
• Once you start the application, you click "Browse + Add Programs to Application"
• You should see Montera Middle School, and click "Add" next to the After School Program
• Then click "Continue to Application" at the top of the page
• You are then able to Submit the Application.
• After you submit, you will receive a notification to fill out an intake form where you can select "Sports only" as the type of after school program. 

STEP 2: Complete the 2026-27 OAL Middle School Sports Participant Registration.

The following forms are required. Please follow the steps outlined below. 

Please click the links below to download and complete. Hard Copies are also available at the Front Office or Athletic Director Office. 

• ENGLISH |  SPANISH  |  CHINESE Combined Physical Evaluation Document: History, Exam, Medical, Parent Permission (PDF for easy printing) 

*Pages 1-3 include the History Form and Physical Exam Form which are to be filled out by the parent/guardian and physician. Copies are to be kept at the physician's office and with parents/guardians. 

*Page 4 is the Medical Eligibility Form. This form will need to be signed and completed by your child's physician. 

*Page 5 is the Parent Permission Form. This form needs to be completed by parent/guardian.

STEP 3: Schedule and complete a physical examination for your child with your child's physician after July 1, 2026

Physicians will need to complete and sign the Medical Eligibility Form and the OAL-OUSD Physical Examination Form.

 ALTERNATIVELY:

• Physicals are available for Montera Families @ $25. Contact Dr. Konstantin's office.

• Students with Kaiser who are limited to one physical per year should contact Kaiser’s Release of Medical Information (ROMI) to request a sports clearance form that covers the duration of their physical.

**Sports Physicals Submission**

All athletes are required to submit sports physicals completed and dated after July 1st of this year. These pages should be scanned or photographed clearly for submission.

Sports Physicals Submission Deadline

PRIOR TO THE START OF THEIR PRACTICE. Students without a Pre-Participation Physical will NOT be allowed to start training/practice. It is imperative that we receive the required documentation by the specified deadline to ensure that your athletes are eligible to participate in the upcoming season.

1. Email the completed Parent Permission and Medical Eligibility Form to the Athletic Director, Mr. Price, at martin.price@ousd.org  
2. Upload the OAL-OUSD Medical Eligibility Form or alternatively Kaiser's sports clearance form to Arbiter Sports. First, create an account on Arbiter Sports, then follow the steps to upload the Medical Eligibility Form and answer all questions/acknowledgements.

Each student-athlete must meet the below requirements to remain in the Sports Program:

• Standard academic eligibility under OAL (2.0 GPA).
• Have an attendance rate of 93% the previous semester.
• Meet behavior standards and have approval from admin/principal.
• Sign in at the MUR prior to 3:45 PM before going to sports practice.
